Perfeccionamiento en la realización del monotúnel y en la colocación del tornillo interferencial en las plastias LCA (HTH)

| La práctica de colocación de
injertos libres HTH por cirugía artroscópica y con el método de túnel cerrado
proximal, o monotúnel, ha alcanzado gran difusión en los últimos 3 años. Para su
perfeccionamiento, hemos estimado unos criterios anatómicos orientativos en la
realización del túnel femoral, según la salida intraarticular del túnel tibial. Una
vez realizado el túnel femoral no se debe variar el ángulo de flexión fémoro-tibial,
introduciéndose el tornillo interferencial por el orificio distal a través de la aguja
guía una vez situada y tensada la plastia en su clavo guía en el túnel ciego. Por los
resultados obtenidos a corto y medio plazo se estima una garantía en la situación de la
plastia y una mejora técnica en la fijación del HTH por cirugía artroscópica con

Improvement in establishing
the monotunnel and in the placement of the interferential screw in ACL (HTH) plasties. The
placement of free HTH implants through arthroscopic surgery and with the method of
proximal closed tunnel, or monotunnel, has achieved considerable diffusion over the past 3
years. For its improvement, we have considered orientative anatomic criteria in
establishing the femoral tunnel, according to the intraarticular outlet of the tibial one.
Once the femoral tunnel has been pierced, the femoro-tibial flexion angle should not be
changed, the interferential screw being inserted through a guide needle once the plasty
has been placed and tensioned on its guide nail in the blind tunnel. Based on the short-
and mid-term results, we consider there is a guarantee in the placement of the plasty and
a technical improvement in the fixation of the HTH through arthroscopic surgery with
